It is the objective of this project to develop a generic telematic homecare platform that enables patients and users at home to be integrated in a secure telecom-munication healthcare provider network. The modular telematic homecare system (THS) back-bone will be suited to meet the home healthcare requirement of different healthcare scenarios suited for the requirements of the German public health system. The modular design of the THS will allow to tailor the telehome-care systems according to the regional configuration of health care providers and networks.

The main emphasis in the first year are technical developments regarding:

  • Realization of a communication server based telematic home-care system (THS)
  • Development of virtual drivers for receiving monitored vital sign and patient data
  • Development of a home based patient record with gateways to various health system standards and secure data exchange.

One field trial will be initially pre-pared concomitant with the tech-nical developments. The field trial will be conducted in Berlin by a group of local healthcare provid-ers. The initial field trial concerns the treatment, rehabilitation and secondary prophylaxis of patients after stroke and congestive heart failure. Measures include ambula-tory, continuous monitoring of blood pressure and body weight. In particular telematic control of blood coagulation will be investi-gated in patients, when treated with anti-coagulant agents. Medi-cation intake adherence (compli-ance) and remote cognitive ther-apy will be studied. The field trial will commence in the second year of the project and will last for at least one year. The field trial will include a group of approximately 20 patients and a health provider network of a call center, a clinic, a rehabilitaion clinic, a nursing ser-vice and 2-3 practitioners. Clinical partners are the Brandenburg Klinik and the Evangelisches Krankenhaus Königin Herzberge.
